Report a Concern or Incident

FRCC is dedicated to the health, safety and well-being of our college community. We encourage faculty, staff and students to report matters regarding discrimination, concerning student behavior, concerns about student well-being, sexual misconduct, student grievance and complaints, and general concerns.

Please use the links below for more information and to submit a report. 

Issue an Alert/Student of Concern 

To report students who are displaying academic or non-academic concern, go to your Navigate360 homepage. Once there, click on “Issue an Alert” located on the right-hand side.  

Types of concern may include poor academic performance, lack of access to essential needs, self-harm and/or suicidal tendencies, concerning behavior, grief, medical concerns, lack of engagement, and need of academic supports (tutoring and Academic Success Center).

This process should NOT be used to report violations of the Code of Conduct

Academic Integrity Reporting Form

The Academic Integrity Reporting Form is to report alleged academic misconduct by one or more FRCC students. The policy for academic integrity/misconduct can be found in the Student Rights and Responsibilities Code of Conduct.

The Incident Reporting/Student Conduct Referral Form is to report alleged non-academic misconduct by one or more FRCC students. The policy for conduct can be found in the Student Rights and Responsibilities Code of Conduct

Harassment/Discrimination/Retaliation Form 

Use this form if you believe that you or others have been unlawfully discriminated against, harassed, retaliated against or otherwise harmed. You are encouraged to fill out this complaint form; however, you can also call Human Resources at 303-678-3707 or email JoAnne Wilkinson to have your complaint investigated.

Student Grievance/Complaint Form

Use this form if you believe you have been subjected to an alleged inequality in violation of the Community College System's Board Policies or System President's Procedures. This Student Grievance/Complaint Procedure is intended to allow students an opportunity to present an issue which they feel warrants action, including the right to secure educational benefits and services.
