Forms for Faculty Use

What services are provided by the testing center for faculty:

• Makeup Exams
• Exams for students with documented extended testing time
• Online Learning Courses
• Hybrid Courses

What services are not provided by the testing center for faculty:

• Retesting--if you choose to offer a retest for a student or class you need to administer the retest
• Entire classes--faculty who need to be absent for an exam need to arrange for a substitute during that class period
• Entire classes due to school closures--if the college is closed during a scheduled exam, the instructor needs to arrange for a different class period to give their exam

All course exams must be administered via Blackboard .  **Math courses are the only exception.
